Covid-19 Information & Consent Form

The third national lockdown has been announced and I have yet again been forced to close. Below is some information on how your booking will be affected once I’m able to work again  

I’m really looking forward to working with you all again but as you can guess there are going to be some changes on the way I will have to work and I have been working hard behind the scenes keeping up to date with the guidelines. I have always tried to work hygienically but now thats going to be stepped up a notch and I have made some tweaks to my kit to make sure I can work as safely as possible. I am going to be updating instagram stories with the changes I have been making to my kit. Once they are up you will be able to see that here: @SarahRussellMUA

Details on how to prepare for your hair/makeup appointment:

• Services will need to be carried out from a wipeable/cleanable table surface (please let me know if this is a problem and I can bring my fold up table)
• I will need to be able to work in a well ventilated area
• I will need access to hand washing facilities (but I also have plenty of hand sanitiser)
• If anyone else is with you or the booking is for more than one person then they need to be able to be 2m away when they are not in my chair
• You will need to wear a face covering.  To allow this to be worn through as much of the appointment as possible for bookings of both hair & makeup the hair will be done before makeup. Hair bookings the face covering will be worn throughout and for makeup you must start the booking with a face covering and I will advise when its time to take it off.
• I will be in both a mask and safety glasses/goggles or visor
• Timings of appointments may run longer due to the extra hygiene measures
• You will need to complete a covid-19 form either by filling in the online form below (within 24 hours of the appointment) or I will also bring hardcopies with me on the day. (basically saying you don’t have symptoms and for track and trace)

I knowingly and willingly consent to having hair / makeup service during the Covid-19 pandemic.

I confirm that I am not presenting any of the following symptoms of Covid-19 listed below: - Temperature above 37.8C or higher - Shortness of breath - Loss of sense of taste or smell - Dry, persistent cough - Sore Throat

I confirm that I have not been around anyone with these symptoms in the past 14 days or that I do not live with anyone who is sick or quarantined.

I confirm that I am not waiting on Covid-19 test results.

I confirm that I have not received a positive Covid-19 test result in the past 10 days.

I confirm that I am not waiting on Covid-19 test results.

I confirm that I have not travelled outside of the UK in the past 14 days.

I agree to all of the above mentioned and agree to proceed with my treatment and will not hold Sarah Russell Hair & Makeup Artist in any way responsible if I was to go onto show any symptoms of the virus.
